Over the years, our Home Care and Hospice programs, Medicare Certified and CHAP Accredited, have grown in their capabilities to meet the needs of our communities. Home Care, once thought of as a service used only after an inpatient hospital stay, is now used more extensively within the outpatient and clinic areas to help patients regain control of their lives and restore a sense of independence within a home setting.
Your Mercy Home Care team of experienced professionals deliver a wide range of services using skilled nursing, home health aides, physical, occupational, speech therapy, social work and nutritional consults to help you achieve the needed recovery or healing from a medical situation or help you address health and lifestyle changes related to new and existing chronic disease diagnosis. To assist patients with specific needs, we offer focused home care programs dealing with heart and cardiac issues, diabetic care, infusion care, wound and fall prevention issues. We also offer specialized Telehealth services.
Hospice is not a place but a specialized end of life care that supports both patients and their families. Mercy Hospice cares for people with life-limiting illnesses. Mercy Hospice patients are able to live with peace and dignity in the comfort of their own homes or residences such as nursing homes.
Mercy Hospice focuses on the needs of patients and families. Mercy Hospice nursing staff works with your physician or in coordination with the hospice physician. The Mercy Hospice team includes registered nurses, social workers, spiritual care, home health aides, music therapist, volunteers, and a variety of therapists that work to enhance quality of life for both the patient and family.
